By filling out the form, I give consent for Ena’s Community Cats to bring my pet to one of our trusted veterinary partners for a spay/neuter procedure. I understand that my cat will be undergoing a surgical procedure, and understand that there may be risks to this procedure and anesthetic episodes.
Ena’s Community Cats, their veterinarian(s) and staff are to use all reasonable precautions against injury, escape or destruction of the animal, but will not be held liable or responsible in any matter whatsoever, in any circumstances, on account of treatment, care or safekeeping of the animal, or otherwise in connection therewith, as it is thoroughly understood that I assume all risks.
In case the animal(s) are not picked up on time, a written notice will be emailed to the address on file to remove the animal(s). Five days after such written notice the animal(s) will be considered abandoned and will be surrendered to a local rescue agency at Ena’s Community Cats’s discretion.
Your cat will be in proximity to other cats both in our program, and at the vet's and should be fully vaccinated for their health and safety.
I hereby certify that I am 19 years of age or older, I have read and fully understand the above conditions of this hospital and acknowledge a copy of this form can be provided to me at any time by request.
